Student Working Papers/ Student Records

1. Click on the Web Working Paper Application
2. Fill out section I and II be sure a parent signs the form
   
3. Get working paper physical and have physician sign Physicial Fitness Certification
4. Take completed application to your team office to receive your working paper card
